On April 29, 2005 the Board of County Commissioners (BCC) approved a ten (10) year lease of Lot 44 parcel number 30-19-31-509-0000-0440 (300 Bay Avenue, Sanford, FL 32771), to Seminole Community Mental Health Center, Inc. On September 4, 2014 the Board of County Commissioners (BCC) approved an Assignment of Lease Agreement from Seminole Community Mental Health Center to Aspire Health Partners, Inc. On March 10, 2015 the Board of Commissioners (BCC) approved the First Amendment to Lease Agreement which extended the lease for a term of five (5) years from April 29, 2015 through April 29, 2020 with an option to renew for an additional five (5) year term.
This proposed Second Amendment and Final Renewal renews the lease from April 30, 2020 to April 29, 2025.The annual lease rate remains at one ($1.00) dollar. All other terms and conditions from the previous lease remain the same. The mailing and notice address is updated per Tenant’s request.
