Background:

The County entered a contract with Image Trend, Inc., Lakeville, MN on September 20, 2011 for the installation and support services of the Image Trend Records Management System (RMS) software for the Fire Department/Fire Rescue. Image Trend software provides the County with patient care data and reports, which allows staff to make clinical decisions that improve patient care and availability of services. This software is utilized by all but one of the Cities' Fire Departments within Seminole County in partnership with the County.

Amendment #7 will provide The Community Health Add-On (Elite Community Health) which is a complete solution for community paramedicine and mobile integrated healthcare (CP/MIH). It allows the use of patient data already collected to identify appropriate candidates for CP/MIH programs, track progress, report on various metrics, and analyze the results. The Community Health Field (Elite Community Health) is the mobile friendly version of the Elite Community stand-alone version.

This is a pilot program. If after 60 days this is a successful program, the Fire Department will move forward with continued service. Amendment #7 adds a total one (1) time fee of $11,200.00 to this Agreement for this pilot program with a $9,000.00 annual recurring fee, for a revised contract total of $35,500.00 recurring annually beginning January 1, 2022 through September 30, 2022. The recurring fees will escalate annually by 3% beginning in year two (2) (2022) and thereafter.  The Term of the Agreement will be through November 5, 2024 with the option of two (2) 1-year renewals.
