Approve the Homeless Partnership Agreement Fiscal Year 2021-2022 - Impact Homelessness Fund in the amount of $143,755 for continued support of the chronically homeless population and authorize the Chairman to execute agreements with the subrecipient agencies as needed. On December 11, 2018, the Board of County Commissioners approved the Central Florida Commission of Homelessness (CFCH) consideration of funding to continue to fund the Chronic Homeless Pilot Project initially funding by Florida Hospital Impact Fund.  The project currently provides housing and supportive services for 28 clients whose homelessness originated in Seminole County. This is renewal funding for the Impact Homelessness Fund.

·         Pathway Homes - $77,145

·         Health Care Center for the Homeless - $37,996

·         Total - $143,755

Each entity will enter into a separate subcontract agreement.
